Often times, it’s a matter of life and death. We’re working to extend a LEISHLine to families in need of medical financial assistance through a strong partnership with Patient Advocate Foundation. Together, we’ve established a fund to serve families suffering from Sepsis, ARDS, and Toxic Shock Syndrome to help offset uninsured expenses. If you’d like more information on how to help, please email Info@beginagainfoundation.com today.
LEISHLine Financial Aid Fund
In partnership with Patient Advocate Foundation, the LEISHLine Financial Aid Fund works to support eligible patients who have an Acute Respiratory Distress Syndrome (ARDS), Sepsis and/or Toxic Shock Syndrome (TSS) physician verified diagnosis within a 12 month look-back period. Each approved patient will receive a one-time $1,000.00 payment to be applied towards the following expenses: rent/mortgage, transportation, utilities, food/nutritional needs and/or lodging.
Each patient/applicant must provide a documented need for financial assistance based on the eligibility guidelines of an annual household income at or below 400% of the Federal Poverty Guidelines (FPG). Proof of income documents may include:
- Copy of a most recent Federal tax return
- Statement of Social Security benefits (i.e.: copy of award letter, copy of check)
- Social service approval for programs such as food stamps, Medicaid or TANF
- Statement of short term/long term disability benefit
- Statement of pension benefits
- Other proof of income (such as 3 months paystubs)
As of July 11, 2019, the LEISHLine Financial Aid Fund is closed to new applications for the remainder of this month. Patients may apply starting the first business day in August and/or request case management services.
or apply online at www.patientadvocate.org/financialaid
Monday-Thursday: 8:30am to 5:00pm EST | Friday: 8:30am to 4pm EST
We will award grants to five approved applicants per month. The application portal will close once we have exhausted all five grants for the month and reopen on the first of the following month. All applicants will be considered on a first come, first serve basis and applications will be reviewed by the Patient Advocate Foundation’s team of certified case workers. Funding requests can be made by phone or online through the LEISHLine Financial Aid Fund.
We will endeavor to accommodate requests where appropriate, however, due to the large volume of requests made each month, we cannot guarantee that all requests will be fulfilled.
